Becoming A Member

How to Become a Member

Applicant has to download and fill up the Membership Application Form.
Applicant is advised to refer to Membership Application Form [How To Fill Up Guide] to assist in filling up the application form.
All relevant fields as per the guide must be completed.
Use of black or blue ink pen is allowed.
One time entrance fee is RM20.00.

Completed application form and RM20.00 one time entrance fee can be submitted through the following channels:

By hand at our office.
By courier to our office address.
- Applicant has to bank in the RM20.00 entrance fee into our Koperasi NUCW Berhad’s bank account.
- Attach the bank in slip together with the completed Membership Application Form when courier to us.
- Please do not send cash through post.
- Please contact our office to obtain our bank account details.

NO	TERMS OF MEMBERSHIP
1	Have attained the age of 18 years at the point of registration for membership
2	A Malaysian citizen
3	Employed or has a source of income
4	Not be a person against whom a conviction stands for a registrable offence or not mentally disabled or not an undischarged bankrupt or has not been expelled from membership of any co-operative society and a period of one year has not elapsed from the date of such expulsion.
