How to Become a Member
- Applicant has to download and fill up the Membership Application Form.
- Applicant is advised to refer to Membership Application Form [How To Fill Up Guide] to assist in filling up the application form.
- All relevant fields as per the guide must be completed.
- Use of black or blue ink pen is allowed.
- One time entrance fee is RM20.00.
Completed application form and RM20.00 one time entrance fee can be submitted through the following channels:
1. By hand at our office.
2. By courier to our office address.
- Applicant has to bank in the RM20.00 entrance fee into our Koperasi NUCW Berhad’s bank account.
- Attach the bank in slip together with the completed Membership Application Form when courier to us.
- Please do not send cash through post.
- Please contact our office to obtain our bank account details.

| 4 | Not be a person against whom a conviction stands for a registrable offence or not mentally disabled or not an undischarged bankrupt or has not been expelled from membership of any co-operative society and a period of one year has not elapsed from the date of such expulsion. |
